- The first time you place an order, you will be asked to enter basic Account information. For all subsequent orders, you will only need to Login, or enter your personal ID (which is your e-mail address) and password. Tile/Plaque/Urn Order Form - This is a detail form that may be helpful to use for your tile, plaque or urn order. This form should be filled out, saved, and attached to your email or included with your fax or mail in order. One form must be filled out for each item ordered.
F-Series Warranty Validation Form - If you own a Ray Allen F-Series Deployment and Heat Alert System, please complete this form and fax to Ray Allen at 719-380-9730. This will validate the 2-year warranty from the date of purchase on your F-Series System.
