approved. Prices are F.O.B.
Colorado Springs, CO (you pay freight). Mail orders must
include shipping and handling. Colorado residents add
3% sales tax and in El Paso County (CO) an additional 1%. |

Tile/Plaque/Urn
Order Form - This is the detail form for
your tile, plaque or urn order. This form may be filled
out online when you order your item, or you may print
this form and fax or mail it in. One form must be filled
out for each item ordered.
Sizing
Chart (Bite Suit & Muzzle)- Use
this form if you need a custom bite suit or muzzle. The
majority of our orders are filled without the use of this
